**Forms above requiring notarization can be notarized during your initial interview.
Second Step:
Proceed to the application process, where you will fill out the online application and attach the above completed forms.

Your application will remain on file for 120 days. We will respond in the order your application was received and will contact you when/if more information is needed.
Third Step:
Before you attend the initial interview, you will need to bring all required documents listed on the Applicant Checklist below.
APPLICANT CHECKLIST – The following items will be required at initial interview.
- Copy of certified Birth Certificate
- High School Transcripts (must be sealed) or Diploma
- Black/White copy of SS Card and NCDL
- If you have criminal charges, we will need a certified Criminal History from counties in which you have lived (Contact the Clerk of Court in each county/state to obtain this information)
- Credit History Check from one of the 3 major credit reporting bureaus reporters (**You only need to provide information about overall credit score and anything showing in collections.)
You can get a free copy of your credit report once every 12 months from each of the three major credit reporting agencies: Equifax, Experian, and TransUnion)

By Mail: Complete the Annual Credit Report Request Form (available for download online or through the mail) and mail it to: Annual Credit Report Request Service, P.O.
